In a workplace death case, investigators and civil lawyers would want to know whether supervisors tolerated horseplay, whether compressed-air equipment was left unsecured or misused regularly, whether workers had training on hose safety, and whether prior incidents or warnings were ignored.<\/p>\n      <p>That broader context is essential.
