Surveillance footage reportedly showed the operator performed the mandatory departure check before leaving the station. But Adams was about 611 feet away from the operator&#8217;s location, and federal investigators said dim lighting at Powell Street Station likely made it difficult to see activity at the far end of the platform.<\/p>\n      <p>Investigators also concluded the leash was too small to be detected by the train&#8217;s door safety mechanisms. That detail is important because it points to a technology gap, not just human reaction time. When a narrow item such as a leash or cord can be trapped without triggering a warning, questions naturally follow about door-sensor design, warning systems, operating assumptions, and whether a transit agency&#8217;s safety setup is adequate for foreseeable real-world hazards.<\/p>\n      <p>Public reporting later said toxicology findings suggested Adams may have been experiencing at least some impairing effects of substance use, but the NTSB reportedly found no clear evidence that impairment played a role in the fatal sequence.
